If medication needs to be taken during the school day, then the appropriate form needs to be completed:-

• If medication needs to adminstered by staff please click here to print off a 'Request to Administer Medication' form.
• If medication can be self-adminstered by the pupil (including inhalers) please click here to print off a 'Request for Child to to Self Administer Medication' form.
